California Medical Sun-Shading Evaluation
FormsMD provides physician telehealth evaluations for eligible California patients who may need medical documentation related to sun-shading or window tint accommodations. Each request is reviewed individually, and documentation is issued only when medically appropriate based on the telehealth evaluation, patient history, and supporting information.

Important California Requirements
This service evaluates whether a medical condition may support documentation for a lawful medical sun-shading accommodation under California law. A physician evaluation or letter does not authorize unrestricted, illegal, or permanently darkened front-window tint. Sun-screening devices may include removable sun shades, removable window screens, or similar removable devices designed to reduce sunlight exposure; these differ from permanently darkened window tint. California law places specific requirements on device type, visibility, and use. Certain clear UV-protective films may fall under separate requirements and may require certification by a licensed dermatologist; this general physician evaluation does not approve those dermatologist-specific exemptions.

What to expect from your telehealth evaluation
During a secure online visit, the physician reviews your relevant health history and the documentation you are requesting. You may be asked to provide prior medical records, a medication list, forms, or other supporting documentation when applicable.
